Background: Hypertension in East Java reaches a prevalence of 32.8% with a low control rate of only 16.3% due to treatment fatigue. This study aimed to prove the clinical effectiveness of music therapy as a community-based complementary intervention on blood pressure reduction among primary hypertension patients in Ardimulyo Health Center. Subjects and Method: This study utilized a quasi-experimental design with a pretest-posttest control group approach. A total of 60 respondents were selected through purposive sampling and evenly distributed into intervention and control groups. The intervention group received instrumental music therapy (tempo 60-80 bpm) for 15 minutes daily over 4 weeks with family and health cadres support. Data were analyzed using Paired Sample T-Test and Independent Sample T-Test. Results: The intervention group showed a highly significant reduction in blood pressure (Systolic: 155.20 to 135.30 mmHg, p=0.000; Diastolic: 96.40 to 85.20 mmHg, p=0.000). Conversely, the control group showed no significant reduction (p>0.05). There was a highly significant difference in the reduction margin between groups (Δ systolic p=0.001; Δ diastolic p=0.001). Conclusion: The integration of music therapy with a community social support system is empirically effective in reducing blood pressure and increasing self-efficacy and compliance in primary hypertension patients compared to sole pharmacotherapy
